Anybody know how this happened?

The Bus didn't seem to be slowing down, but attempted to swerve and not hit that first car.

RIP to the lives lost in the crash.

looks like brake failure to me...sounds like the bus driver was trying to warn the first van by honking but ended up swerving anyways.
damn wonder how many whiplashes occurred in that crash.

why is that? does this sort of thing happen frequently enough to require it?

(i know everything is split second but if only the driver had gone right instead of left and used the emergency shoulder and maybe ride the barrier to slow down...)
