Todd tells Walt, Skyler is dead, and if he tries to escape, Walt jr & holly are dead.
Walt teams up with jesse, does the smoke show thing again when they killed amelio and suffocated cray 8.

They escape together, but Todd's still alive (because he practices safety, remember that episode where he goes "uncle, wear your mask"). Out in the dessert, Jesse turns his back on Walt, because Walt caused all this shit and death, shoots Walter in the head. Walter falls down like ozymandis, but this time he's dead.

Something along the lines of this, is my guess.
