people who don't wear seatbelts are only hurting themselves. probably will cause PTSD for a witness or another driver who sees them being propelled through the windshield, but it is the person not wearing the seatbelt that will be dead and/or crippled.
People have actually been killed because the body of the person not wearing a seatbelt slammed into them. Would you want to be hit by a big sack of meat during a car crash?

I am sure that is a possibility just like anything else. But I wouldn't bet on that being the fatal factor in the vast majority of MVIs
