Not necessarily. I've been chatting with this guy on
www.thephotoforum.com who uses a 580ex flash and a Lumiquest Softbox for a diffuser. His photos are amazing. Check out his website:

He doesn't use a ring flash at all and 98% of his shots are handheld.
