I think there is a time limit. Not exactly sure, but the questions on the form goes someting like, "Have you had a tattoo in the last six months?"
I love the questions they ask..........short of asking you if you had sex with a frickin' monkey. "Have you come in contact or work with monkeys or their bodily fluids?" I chuckle each time I come across that one.
