Don't do it that way.
Get a USB 2.5" drive and copy the files over and take that with you. You don't want to be carrying your only copy of the data around.
It's not possible to boot up in the way you want since the hardware is totally different. Unless your home pc and work pc are exactly the same. Still not recommended though.
