The idea of spending money for in-game stuff may be new to many especially since no game has incorporated it like D3 but its more common than you think. Lots of gamers buy gold, items, and even SKINS(look at League of Legends, it only changes the looks of characters). People buy "content packs" all the time I dont really see much difference. They both enhance gameplay experience for the player.
Its also an investment. My friend spent $500 in D3 so he can use the items to farm in inferno faster. He sold over $1000 so far. For him ,its a "summer job" lol. This was pre-patch though so I dont know how well he is doing now
