So a shop can't "milk" the system, because the adjuster would have to have seen the damage.

No, this is not true.
I was rear ended a month ago and after describing the situation to the adjuster, he gave me a ICBC express claim number. I just went to a body shop and it was pre-approved. Mind you, it was pretty clear the other person was 100% at fault. She hadn't even called in though, so I was pretty surprised they pre-approved my claim.
