I pretty much agree with everything posted here.

As an employer, I wouldn't want someone unreliable working for me. I would do everything within my power to, legally, get rid of you. The best thing you could do for me is to quit.

As Spoon Fed said, see if you can go on medical leave to get yourself sorted out. It's the best thing for your health and if they can't get rid of you, for your employer.
