So the woman is suppose to be brooke (rat's gf) mom. Took a few notes of reddit which I think sums it up pretty good.

a)I believe its a reference to the Last Supper. When the homeless woman gave him the blanket, the camera panned onto a bottle, which looked like it contained blood, along with bread. These two items are given to his (Jesus) disciples when he knows what Judas had done to him, along with his likely end.

Then, in the end, we see the crows eating the bread, and Jax's blood come onto the screen. This being after his sacrifice to remove the "original sin" from his sons, who had he stayed alive, would have continued into the club life.

b)She's the Reaper, appearing in the form of Brooke's dead mother. She's always around when someone dies, from Donna, to Gemma, to Jax himself.

The bread and wine was obvious. But in the VERY next scene. Jax looks an AWFUL lot like the SOA Grim Reaper logo when he kills August Marks at the courthouse.

3) Emily Putner appears several times in Sons of Anarchy. Always when Gemma or Jax are going through an emotional decision making process. In the episode 'John 8:32' it was stated that she is Emily Putner, the mother of Brooke Putner. Emily was accidentally killed as a result of John Teller's collision with a semi-truck and trailer in 1993. In the same episode, when Jax goes to talk to Gary Putner about his daughter's aggressive behavior, he sees a wedding photo of Emily with Gary. When Gary asks Jax why he's curious about it, he states that "She looks familiar". Gary then says that he has also often seen her. He also tells Jax that he has had other people come to him saying that they have seen her too.
