Yes, 2 cream and 2 sugar. Its ratio'd out so that a small double double tastes exactly the same as an extra-large. It's just the total volume of beverage that changes.
Many consider it to be gross and unhealthy due to all the cream and sugar involved (admittedly, it is a lot of sugar in a large). But consider this: In Ontario the Triple-Triple is very common and in some areas (Hamilton area mostly) the 4x4 is well-known and popular! Gross!
