Step 1:

Step 3: Purchase pre-order pack from within hearthstone app, It will say it costs 4999 coins (but it actually costs 4999-1500 coins = 3499 THIS IS YMMV) -> you will also receive 1000 coins back!

Help for Getting your Amazon Account "Americanized":
-amazon.com->Your Account->Manage Address Book->Delete All current addresses->Enter A New Address-> enter random legit US address (use a state with no tax such as Delaware. I used Newark, Delaware)

steps that led to success from another user (Thank you Unicorny):
I had an issue where when I checked out on Hearthstone, it only gave me the option to pay in CDN. But I found a way to purchase with coins on an existing amazon account that is Canadian. What I did was:
1) Delete my address, add in a US one.
2) Went to Your Account -> Your Apps and Devices, then deleted the app and my devices (On the Canadian Site)
3) Went to Your Account -> Manage Your Content and Devices -> Settings, then changed the Country from Canada to United States
4) Redownloaded Hearthstone
Then it worked for me

Whew, finally got it to work. I got 1000 coins back as well...
- Got the $50 USD gift card using a Canadian CC with Canadian billing address
- Switched country to US through "Manage devices"
- Deleted all addresses and payment methods
- Re-added Canadian CC, set billing address to a fake one in Oregon
- Bought the 5000 coins for $40 USD
Here's how I got the cards on my Windows 10 machine:
1) Install Duos Lolipop:

2) Launch Amazon App store (preinstalled) and download Hearthstone. You need to enable unknown sources in Settings in order to install it.
3) Use a VPN service in order to get a US IP. I used Hotspot VPN trial:
