I'm not a designer, but I appreciate designers/artists. Similarly, with logo design, there are apps with preloaded assets and there are custom designed logos. Assuming that process for commissions work the same way, you're paying for a one-off/customization + revisions. In both logo design and commissions, the designer should work with you from begining to end, ultimately creating a piece of art that you both put work into. The designer's job is to put your dreams and vision on paper.
I guess in layman's terms, the app gives you a cookie cutter design of your car. Whereas the artist would add their artistic vision and style to compliment your car.
Or in car terms, you can buy a car and then you add Spoon parts to it (app - cookie cutter/everyone has access) or you can buy the car and get things custom fabbed to how you want it but within the limitation of the engineer (designer).
