Can't speak for everyone, but I had this happen to me after I passed a phone screen, they flew me out last minute to SF, put me up in a nice $500 suite for a night, and did a whole day on-site interview.

One possibility is a direct referral to the hiring manager? I definitely know some roles that continue to hire less qualified candidates because of direct referrals..
