The simplest thing to do is to talk to your HR dept, or whoever is the equivalent of your HR dept. If it is a sizable company or a unionized one, there will 100% be an established process already in place, and you just have to follow the procedure.
If it is a small business, then you'd probably have access to your boss anyway, and it is his job to figure out what he needs to do. But mostly, it revolves around your work place issuing you an ROE (and filing it with CRA). After that, you'll need to sign up with Service Canada for parental / maternity leave benefits because it is paid through the EI system, except that you don't need to file any reports with Service Canada.
If your company offers any salary top ups, your HR dept will have the standard procedures ready, and it'll most likely be a top up over the amounts EI pays you.
